Transition Planning

The JMSA provides for a smooth transition for students entering and leaving the JMSA School Program. The JMCC School recognizes that services to families must address the multiple changing and intertwining needs of children, families, and community service providers. The following strategies have been implemented to assist families making decisions at crucial crossroads of their child’s educational and rehabilitative needs. The premise underpinning coordination of services is based on the recognition that pediatric rehabilitation services can be overwhelming for families. Consequently, clear, defined plans must be articulated.

A. Transition from JMCC Outpatient Services to John McGivney School:
For children receiving JMCC therapy services, who are school-aged and met the eligibility criteria for admission to the John McGivney School, the following processes occur, with parent consent:

1. Referral
2. Collation of Assessments 
• All appropriate data including medical and other assessments are procured.
3. Multidisciplinary Team Meeting
• Family, rehabilitation team and educators meet to determine the suitability of the program and student’s needs.
• A Release of Information Form is completed.
4. “School Bell is Ringing” Parent Education Workshop (one per year)
• A workshop to orient parents on the transition process.
5. School Visit
•  Student and family visit school and meet staff.
6. Initial Registration

B. Transition from JMCC Outpatient Services to District School Board:
For children receiving JMCC therapy services, who are school-aged, the following processes occur, with parent consent:

1. Multidisciplinary Team Meeting
• Family, rehabilitation team and educators meet to determine the suitability of the program and student’s needs.
• A Release of Information Form is completed.
2. “School Bell is Ringing” Parent Educational Workshop (one per year)
• A workshop to orient parents on the transition process.
3. Information Sharing
• Written referral to district school board; referral to CCAC.   
4. Visit by District School Board Personnel
5. School Orientation by JMSA Staff 
6. School Visit
• Student and family visit school and meet staff.
7. Follow-up Support

Ongoing dialogue and collaboration occurs throughout this process to ensure families and District School Board personnel have all available information.

C. Transition/Demission Process from John McGivney School to District School Board:
For children attending the John McGivney School, the following processes occur, with parent consent:

1. Multidisciplinary Team Meeting
• Professional team review to determine student progress with respect to possible transition.
2. School Team Meeting
• Team meeting to discuss community school options with parents.
3. Information Sharing
• Written referrals to district school boards and CCAC.
4. SEA Process/Purchase     
5. District School Board Personnel Visit to John McGivney School
6. Transition Meeting
• JMSA and JMCC personnel, CCAC, family and district school board staff attend.
7. School Visit   
8. In-Service to District School Board (as needed)
9. Follow-up Support                                
 
While ongoing dialogue and collaboration occurs throughout this process, frequency is determined by family and student needs. At any time, families can request further information or visits. All appropriate Releases of Information are procured from families before the transmission of information.

The JMCC School principal is responsible to ensure these processes occur and that families understand the transition process.
