Transition Planning

The JMCCSA provides for a smooth transition for students entering and leaving the JMCC School Program. The JMCC School recognizes that services to families must address the multiple, changing and intertwining needs of children, families and community service providers. The following strategies have been implemented to assist families making decisions at crucial crossroads of their child’s educational and rehabilitative needs. The premise underpinning coordination of services is based on the recognition that pediatric rehabilitation services can be overwhelming for families. Consequently, clear, defined plans must be articulated.
